Patient Rights & Responsibilities

Each patient treated by Grow Through Life Counseling has the right to:

  • Be given verbal and written notice of rights and responsibilities in a language and manner that ensures understanding by the patient or their representative.

  • Be treated courteously, with respect and dignity, free from discrimination or reprisal.

  • Receive care services without discrimination based on race, color, national origin, age, religion, disability, sex (including pregnancy, sexual orientation, gender identity), or source of payment.

  • Have access to interpretation services whenever possible for limited English proficiency.

  • Receive accurate information regarding the qualifications and capabilities of the organization.

  • Receive understandable information to provide informed consent or to refuse care prior to treatment.

  • Be fully informed about treatment or procedures and expected outcomes before they are performed.

  • Receive good faith estimates prior to treatment.

  • Understand services, fees, and payment policies.

  • Have personal health information kept confidential and understand how it is used and shared.

  • Receive care in a safe setting by qualified professionals free from abuse or harassment.

  • Be informed of supervising clinicians when applicable.

  • Request information on provider credentials.

  • Receive treatment aligned with current clinical standards.

  • Decline or discontinue care at any time.

  • Receive appropriate referrals, follow-up, and continuity of care.

  • Share feedback or complaints and receive guidance on resolution processes.

Each patient treated by Grow Through Life Counseling has the responsibility to:

  • Be respectful and considerate of clinicians, staff, and other clients, including arriving on time and communicating cancellations.

  • Refrain from verbally abusive or threatening behavior.

  • Not record sessions without consent.

  • Review and understand all consent forms and ask questions when needed.

  • Request accommodations if needed (e.g., interpretation services).

  • Actively participate in care and communicate openly with providers.

  • Follow agreed-upon treatment plans and discuss any needed changes.

  • Provide accurate and complete health information, including medications and history.

  • Provide insurance and billing information and understand benefits.

  • Meet financial obligations for services received.

  • Respect the confidentiality of other clients.

  • Respect clinic property and notify staff if discontinuing care.
